Every five years, the Oak Park Public Health Department conducts a community health needs assessment and uses the results to create a community health plan, also known as the IPLAN.


The community-wide plan is carried out by the local health system, which includes the Public Health Department, local health care providers, social services agencies and community organizations.

In 2022, the department conducted its most recent assessment and created its current IPLAN. The department is now creating an implementation road map. Better decisions are made when our community provides input, and we want to make that easy. Share your ideas and take the opportunity to lend your expertise and lived experience to our plan.
