Every five years, the Oak Park Public Health Department conducts a community health needs assessment and uses the results to create a community health plan, also known as the IPLAN.


The community-wide plan is carried out by the local health system, which includes the Public Health Department, local health care providers, social services agencies and community organizations.

In 2022, the department conducted its most recent assessment and created its current IPLAN. The department is now creating an implementation road map. Better decisions are made when our community provides input, and we want to make that easy. Share your ideas and take the opportunity to lend your expertise and lived experience to our plan.     The Public Health Department is hosting an open-house style community engagement meeting June 7 from 5:30 p.m. to 7:30 p.m. in the Veteran's Room at the main branch of the Oak Park Library, 834 Lake Street.

    Brief remarks will be given around 6:15 p.m., but Public Health Department staff will be available at individual stations at any time between 5:30 and 7:30 to walk participants through priority areas and answer questions.
