Every five years, the Oak Park Public Health Department conducts a community health needs assessment and uses the results to create a community health plan, also known as the IPLAN.
The community-wide plan is carried out by the local health system, which includes the Public Health Department, local health care providers, social services agencies and community organizations.
In 2022, the department conducted its most recent assessment and created its current IPLAN. The department is now creating an implementation road map.
